Patients are required to wear masks while on-site
In compliance with state and local ordinances, all individuals are required to wear face masks while onsite. All patients will need to wear a face mask or facial protection that covers both mouth and nose in order to enter the facility.
Our Center will continue our guest limitation policy
In observance of social distancing and to minimize the number of individuals on-site, only the scheduled patient will be admitted to the facility. We ask the responsible adult to please wait in his/her vehicle until the patient is discharged. Children are not permitted on site. We apologize for any inconvenience; however, we are committed to protecting the safety of our patients and staff by limiting the number of individuals in our space.
All patients will be verbally screened for COVID-19 symptoms upon arrival at the clinic and well as a temperature check.
Please remain aware of your personal health status and monitor yourself for the following COVID-19 symptoms before coming to the Center for your appointment:
- Fever or chills
- Cough
- Shortness of breath or difficulty breathing
- Fatigue
- Muscle or body aches
- Headache
- New loss of taste or smell
- Sore throat
- Congestion or runny nose
- Nausea or vomiting
- Diarrhea
